You are receiving this form because you had a missed appointment during lockdown or you have registered via our day after lockdown booking request. Given the extended lockdown we are requesting you update your preferences to ensure a smoother booking process.
We also want to give you the opportunity to agree to our covid safety plans prior to confirming your booking.
The current public health order requires all clients and staff in hair and beauty salons to be fully vaccinated. There will be no discussion entered into regarding this policy. We ask all clients to provide proof of vaccination before entering the salon.

DeclarationI confirm that I have had 2 doses of an approved covid-19 vaccine. I agree to providing proof of vaccination upon arrival. I understand that I will forfeit my appointment and booking fee if I cannot provide proof of vaccination.
Please note, the salon will also be trialing Rapid Antigen Tests pior to each appointment. We are doing this to ensure the health and safety of all guests and to keep our doors open. We have a huge list of clients to get through and cannot afford to be closed due to a close contact. We hope you understand and respect this decision.
Please note that mask wearing is mandatory in all treatments until 80% double vaccination target is met. For this reason there will be no lower face waxing, facials, lip filler or advanced body treatments until this time - it is estimated to be 25th October.
Due to the high volume of appointments, and the heavy restrictions on our industry we are offering packages to ensure we can service as many guests as possible in a safe and fair way. In some situations at the salons discretion we may offer additional services to clients where these packages are not appropriate.
Prices are an indication, we have tried to estimate additional product due to extra long regrowth. Product surcharges may apply for longer or thicker hair.
Have you coloured your hair at home? No judgement, we just need to know in advance so we can book your service appropriately. We would hate to have to turn you away if we don't have the time to complete your service.
